Facility ID Number (FID) \u2013 ID number assigned to the underground storage tank facility.<\/SPAN><\/P>

LUST Incident Number \u2013 ID number assigned to the incident when the tank contents were released <\/SPAN><\/P>

into the environment.<\/SPAN><\/P>

Priority - IDEM assigns a priority to each site to ensure that the ones with the greatest chance of impacting people are cleaned up first. The priority is based on information submitted by the <\/SPAN><\/P>

responsible party and/or their consultant.<\/SPAN><\/P>

\u2022 High priority sites have one or more of these conditions:<\/SPAN><\/P>

o Measurable free product<\/SPAN><\/P>

o Drinking water impacts<\/SPAN><\/P>

o Surface impacts<\/SPAN><\/P>

o Vapors in buildings or utilities<\/SPAN><\/P>

\u2022 Medium priority sites have no high priority conditions, but there is possible groundwater contamination.<\/SPAN><\/P>

\u2022 Low priority sites only have soil contamination.<\/SPAN><\/P>

\u2022 Unknown priority means there is not enough information to make a priority determination.<\/SPAN><\/P>

Disposition<\/SPAN><\/P>

Open:<\/SPAN><\/P>

\u2022 Active \u2013 The LUST is currently undergoing site characterization or corrective action.<\/SPAN><\/P>

\u2022 Monitored natural attenuation \u2013 The LUST is approved to allow corrective <\/SPAN><\/P>

action to happen through natural processes, with monitoring.<\/SPAN><\/P>

\u2022 Discontinued \u2013 Discontinued sites are still active, but IDEM designates them <\/SPAN><\/P>

as \"discontinued\" for different reasons, including:<\/SPAN><\/P>

\u2022 The site was referred to another program because the release is not <\/SPAN><\/P>

from a regulated UST.<\/SPAN><\/P>

\u2022 The facility identification number and the LUST number were <\/SPAN><\/P>

assigned based on a complaint, and not to a known facility.<\/SPAN><\/P>

\u2022 The LUST is a dead end because the owner cannot be located, and it <\/SPAN><\/P>

is a lower priority based on site information and potential threats to <\/SPAN><\/P>

human health and the environment.<\/SPAN><\/P>

Closed:<\/SPAN><\/P>

\u2022 NFA - Conditional Closure \u2013 The site was granted a \u201cno further action (NFA)\u201d because it was closed with an environmental restrictive covenant.<\/SPAN><\/P>

\u2022 NFA - Unconditional Closure \u2013 The site was granted an NFA because it was closed without property use restrictions.<\/SPAN><\/P>

\u2022 NFA - VRP 96 \u2013 The site was granted an NFA because it was closed utilizing 1996 Voluntary Remediation Program guidance.<\/SPAN><\/P>

\u2022 NFA - Spill Recovery Response \u2013 The site was granted an NFA because the spill was adequately addressed and did not affect subsurface soils or ground water.<\/SPAN><\/P>

Other:<\/SPAN><\/P>

\u2022 Deactivated \u2013 A facility reported a suspected release but later confirmed that <\/SPAN><\/P>

no release occurred. These are not LUSTs.<\/SPAN><\/P>

\u2022 Complaint \u2013 A complaint was made regarding suspected contamination, but no Notification for USTs was submitted, and a source is unknown.<\/SPAN><\/P>
